First Time Passport Applicants

Requirements:

  • Personal Appearance is required.
  • Duly accomplished application form.
  • NSO Certified Birth Certificate or Certified True Copy of Birth Certificate issued by the Local Civil Registrar duly authorized by NSO.
  • Three (3) colored photos*
    • Royal blue background and No shadow
    • Applicant should be in decent attire with collar & sleeves.
    • Photo must have been taken within the last six (6) months.
    • Photo size: 4.5 cm x 3.5 cm.

    *DFA has the right to reject photos that do not comply with the above requirements or meet standards.

  • Original and Official ID / any two of the following :
    • digitized government-issued IDs like SSS, PRC, BIR, Driver's License
    • original school ID (for students only)
  • If late registered, submit 2 original supporting documents indicating full name, date and place of birth and citizenship prior to date of late registration.


Additional Requirements:

For changing of status from single to married: CHANGE OF SURNAME

  • NSO Certified Marriage Contract
  • Certificate of Attendance from CFO (Commission of Filipino Overseas) if married to a foreigner (color green).

For Minors (below 18 years old)

  • Personal appearance of both minor and mother.
  • Passport of person traveling with the minor, original and photocopy.

If both parents are abroad:

  • Affidavit of support and consent indicating the name of the traveling companion and the relationship to the minor (must be authenticated by the nearest Philippine Embassy or Consulate General)
  • Special Power of Attorney designating the representative by name and authorizing him/her to apply for a passport on behalf of the minor in their stead (must be authenticated by the nearest Philippine Embassy or Consulate General).
  • Certified True Copy of parents' passport (must be authenticated by the nearest Philippine Embassy or Consulate General)
  • Original and photocopy of the passport of person traveling with the minor.
  • Original valid ID w/ signature or passport of the authorized representative.


LIST OF OTHER SUPPORTING DOCUMENTS (as needed only)

  • PRC / IBP ID
  • Government Service Record
  • School Form 137 or Transcript of Records
  • Other documents that show full name and birth details of applicant and/or citizenship
  • Baptismal Certificate

Requirements For Passport Renewal

Green Passport (Passports issued after 01 May 1995)

  • Personal Appearance is required.
  • Current Passport (Original Passport required)
  • Three (3) colored photos*
    • Royal blue background and No shadow
    • Applicant should be in decent attire with collar & sleeves.
    • Photo must have been taken within the last six (6) months.
    • Photo size: 4.5 cm x 3.5 cm.

    *DFA has the right to reject photos that do not comply with the above requirements or meet standards



Brown Passport (Passports issued before 01 May 1995) /
or Mutilated Passports

  • Personal Appearance is required.
  • Original Brown Passport
  • Three (3) colored photos*
    • Royal blue background and No shadow
    • Applicant should be in decent attire with collar & sleeves.
    • Photo must have been taken within the last six (6) months.
    • Photo size: 4.5 cm x 3.5 cm.

*DFA has the right to reject photos that do not comply with the above requirements or meet standards

  • NSO Certified Birth Certificate


Requirements for Lost Passports:

  • Personal appearance is required.
  • Notarized Affidavit of Loss.
  • Police Report – if lost passport is still valid
  • Submit same documents required for first time passport applicants.

Additional Requirements:
